The population is composed of patients hospitalized with ischemic stroke and intracerebral hemorrhage in British Columbia, Ontario, and Nova Scotia who have valid health card numbers in these provinces.

Sociodemographic information such as sex, and age are gathered. Information from the hospitalization, ED visit(s) leading to the hospitalization and associated physician claims are collected and a Passive Surveillance Stroke Severity (PaSSV) score is calculated. Outcomes such as Entry to LTC (Long-Term Care) or death are recorded. Information are collected from the Emergency department Canadian Triages and Acuity Scale (CTAS) such as urgent and non urgent and resuscitation station are collected from the NACRS database. Admition to abulance, Mechanical ventilation usage within 2 days of presentation, transfer marker to a hospital with a higher-level of care and level of consciousness are collected from the NACRS (National Ambulatory Care Reporting System), OHIP (Ontario Health Insurance Plan) and the DAD (Discharge Abstract Database) databases. Speech, visual, motor, ataxia and sensory symptoms are collected from the NACRS and the DAD databases.
