Olmsted County residents aged 70 to 89 years on October 1st 2004, were randomly selected from an enumeration of the County population using an age and sex stratified sampling scheme. Participants return for follow-up evaluations every 12-15 months on average.

Replenishment of the population was ongoing to ensure an active cohort of about 3000 participants.

The in-person evaluation was performed at the Mayo Clinic Abigail Van Buren Alzheimer's Disease Research Clinic or at the participant's residence using a standardized protocol.

Each participant received an evaluation by a study coordinator, who collected information regarding demographic characteristics, medical history, family history, and medications. Blood draw was performed on all participants. Blood pressure, height, weight and frailty were measured.

The study coordinator also interviewed a partner about the study participant and completed a modified Clinical Dementia Rating to assess the cognition and functional activities of the participant.

A physician performed a medical history review, mental status examination, and neurological examination.

A neuropsychological evaluation with 9 tests was performed, comprising 4 cognitive domains (executive function, language, memory and visuospatial).

Subjects who refused the in-person interview were invited to participate in a structured telephone interview by trained study assistants to collect information on education, occupation, marital status and residence, and to assess cognitive function.
