Collected Dataset / HCRS_Home_Care_Assessment

The population is composed of individuals who access publicly funded home care services. These services range from short-term such as a single service in response to a specific event (e.g., nursing care following a stay in an acute care hospital) to long-term needs for support from a range of health providers to remain living in a community setting.

Information is collected about personal characteristics and demographics, social, emotional, and physical health, activities of daily living, and medications. Until 2011, information is collected about prescription medication descriptions from home care assessments. Until 2018, information is also collected about home care assessment risk (at intake, discharge, etc), assessment cases (open and close dates, reasons for closing), facility addresses, home care assessment plans (objectives, control record), home care transactions (prior cases, assessment, and discharge), and provider data (time and days of services provided, tasks, duration of services).
