There are two parts to this study:
* Part I: Sewage SARS-CoV-2 surveillance as early warning system and public health response;
* Part II: COVID-19 Immunity Study
The objectives of this study are to:
* Study the utility and feasibility of long-term care facilities (LTCF) site-specific sewage surveillance of SARS-CoV-2 as an early warning system for COVID-19 that is coupled with rapid Public Health notification and response to prevent outbreaks in LTCF. (Part I)
* Do the cost-benefit analysis of this early warning system. (Part I)
* Study antibody response to COVID-19 among staff and residents of LTCF, stratified by their exposure to and history of infection with COVID-19 during outbreaks and immunizations. (Part II)
* Evaluate the performance of multiple commercial assays to detect vaccine-induced antibodies and correlation with neutralizing antibody responses. (Part II)

- In Part I of the study, sewage samples from LTCF are monitored twice per week for SARS-CoV-2. If the sewage sample is tested negative, then no action will be taken. Positive sewage results may trigger investigations as deemed appropriate by Public Health. In Part II, staff and residents of different LTCF, some with and some without sewage monitoring, were enrolled for an immunity study were followed up at various time points pre-vaccine, pre-second dose, 3-, 6-, 12- and 18-months post second dose for antibodies testing.

The population is composed of residents, staff, and visitors of LTCFs, from where sewage samples tested positive for SARS-CoV-2, and where there weren't recent or active COVID-19 cases.

Ten stand-alone LTCF with various histories of COVID-19 outbreaks and feasible access to site-specific sewage were selected for the sewage surveillance study after review by EPCOR, the lead MOH of Edmonton zone and the Medical Director of Continuing Care, Edmonton zone. Participants were selected based on the positive SARS-CoV-2 water sample at the LTCFs.

Data Collection Event
Based on the positive SARS-CoV-2 screening, information is collected on the date of collection, any action initiated by public health after the report of the sewage results, any change to outbreak start-date and enddate as related to the SARS-CoV-2 results, number and duration of general COVID-19 outbreaks occurred in other LTCF (without sitespecific sewage surveillance), costs of the sewage sampling, testing, reporting and costs of targeted public health actions.
Information on characteristics such as number of residents and category of designated support living, proportion of residents who are diaper-dependent, number of staff, total number of staff working at the site, number of rooms with single accommodation and shared accommodation, private vs non-private providers is collected. Further, information on COVID-19 outbreak history from the start of pandemic January 2020 to the end of the 18-month study. Moreover, information such as time frame, type of vaccine administered and % coverage of the staff and residents in the vaccination roll-out at the LTCF is also collected.
COVID-19 screening of residents, staff and visitors was completed.
