Five follow-up assessments scheduled every 18 months. A final long-term follow-up was conducted 8 years after follow-up 5, for a total observation period of 16 years.

Institutionalized individuals were included in the study by proportion, because in the study area residential homes were over-represented and nursing homes were underrepresented.

Supplementary Information
Participants were identified by systematic random sampling from an age-ordered list provided by the official registry office for each of the seven sub-districts. Institutionalized individuals were included in the study by proportion. Sixty residential home residents and 132 nursing home residents were identified by systematic random sampling from an age-ordered list provided by the four institutions in the study area.
