Participants were selected by stratified cluster sampling of the non-institutionalized adult population of Spain. The sample was first stratified by province and municipality size. Clusters were then randomly selected in two stages: municipalities and census sections. Finally, households within each section were selected by random telephone dialing. Individuals within households were selected proportionally to the age and sex distribution of the Spanish population.
