Identification of Underlying Factors Influencing the Immune Response to SARS-CoV-2 among Workers and Residents in Long-term Care Homes: A Multi-province Study

The overall objective of this study is to determine the humoral, sociodemographic and health-related risk factors that increase the likelihood of a SARS-CoV-2 infection, potential re-infection, and serious outcomes (including hospitalization and death) from COVID-19 among workers and residents in long-term care (LTC) homes. The specific objectives of this study are to:
* Create a multi-province study cohort;
* Measure serological markers in LTC workers and residents from dried blood spots (DBS) samples and compare the immune response in individuals with natural versus vaccine-acquired immunity;
* Explore the humoral immunity in vaccinated LTC workers, residents, and family members/caregivers of LTC residents against emerging variants of concern (VOCs). Further ability to neutralize VOCs or detect VOC receptor binding domains in a standard enzyme-linked immunosorbent assay (ELISA) will be assessed;
* Link data on virus neutralization and serological markers, vaccination records, and key social determinants of health using a survey to population-level healthcare administrative databases such as L'institut de la Statistique du Québec (ISQ), Institute for Clinical Evaluative Sciences (ICES) in Ontario, and Population Data BC to examine long-term health and healthcare outcomes following vaccination;
* Examine the association between humoral immunity (both natural and vaccine-acquired immunity), sociodemographic, health, and workplace factors with incidence of SARS-CoV-2 infections, as well as re-infections, ascertained through provincial COVID-19 testing data, and their influences on outcomes following a SARS-CoV-2 infection, including the odds of hospitalization, admission to an intensive care unit (ICU), use of mechanical ventilation, and death;
* Use regression and machine learning approaches to develop prediction models for SARS-CoV-2 infection, potential re-infection, and severe adverse outcomes of COVID-19, based on risk factors identified in Objectives 3 and 4.

- Participants complete questionnaires and provide a DBS sample first month post 3rd and/or 4th dose administration at the first timepoint and are followed-up every 3 months for a period of 18 months.

- Recruitment in Ontario and British Columbia will include LTC homes that have and have not experienced a COVID-19 outbreak.
