Three municipalities, Chofu, Tondabayashi and Hiroshima, were added during the third collect of information and participants aged between 50 and 75 years were recruited. Chofu is a city of the suburbs area of the Tokyo metropolis, Tondabayashi is a city in Osaka, and Hiroshima is a city in the Chugoku area.

The survey was composed of two parts: a self-completion questionnaire (SCQ) and a computer-assisted personal interviewing (CAPI) section. The SCQ included questions about lifestyle and health, and the CAPI collected information about the individual and his family, memory, cognitive ability and hypothetical questions, employment, health of respondent and spouse, income and consumption, housing and assets, and use of and expenditure on Medicare care and nursing care services.

Grip strength was also measured.
