The individuals in the baseline sample of JSTAR are aged between 50 and 75 and live in five municipalities mainly in the eastern area of Japan. The cities are Takikawa city in Hokkaido, Sendai city in the Tohoku area, Adachi ward which is a special city in the center of the Tokyo metropolis, Kanazawa city in the Hokuriku area and Shirakawa town in a mountainous town in the Chubu area.

The survey was composed of two parts: a self-completion questionnaire (SCQ) and a computer-assisted personal interviewing (CAPI) section. The SCQ included questions about lifestyle and health, and the CAPI collected information about the individual and his family, memory, cognitive ability and hypothetical questions, employment, health of respondent and spouse, income and consumption, housing and assets, moving home, and use of and expenditure on Medicare care and nursing care services.

Grip strength was also measured.
